Position Summary

Under the direction of a Provider and in conjunction with other health care team members, the Care Manager will be accountable for the full adult and pediatric patient care cycle.  We are looking for a care manager to enhance the quality of patient management, maximize satisfaction and promote healthy living.  The Care Manager will conduct comprehensive assessments in order to obtain the most accurate information to build off of the identified needs and risk factors for the patient.

Job Duties:

  • Assessing, planning, implementing, monitoring and evaluating actions required to meet the patient’s health care needs
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
  • Assist patients with making informed decisions regarding their clinical status and treatment options
  • Incorporate current patient literature regarding effective engagement, communication, care management and behavior change strategies and clinical practice
  • Implements evidence-based care, chronic disease protocols and guidelines.
  • Identify improvement opportunities in patient care and coordinates resolution protocols
  • Coordinates patient care by linking patients to internal and external community resources.
  • Collaborates with the primary care provider, patient, and members of the health care team to assess, develop and implement a patient care plan.

Minimum Requirements

Knowledge equivalent to that which normally would be acquired by completion of a four-year college degree program in a Social Work; one to three years of related and progressively more responsible or expansive work experience in care management including chronic conditions, evidence-based guidelines, prevention, wellness, health risk assessment, and patient education; and/or experience with adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical; knowledge of electronic medical records; or an equivalent combination of education and experience. Requires possession of a current license to practice as a Registered Nurse in the State of Michigan, OR completion of a master's degree in social work and a current license to practice as a Social Worker in the State of Michigan.

Desired Qualifications

  • Bachelor's degree in social sciences, social work, nursing, or a related field.
  • At least 2 years of experience in public health or a related field.
  • At least 1 year of experience in case management.
  • Proficient in Healthcare Management Systems and Microsoft Office Word and Excel.
  • Excellent written and verbal communication skills.
  • Strong organizational skills.
  • Ability to travel locally.

The job requires significant physical effort and involves lifting and maneuvering patients, extensive walking and standing and fine manual dexterity 26-75% of the time; visual and aural acuity necessary to monitor patient care.

Exposure to human blood, serum, and other body fluids; and materials covered under Universal Precautions; TB risk or work within three (3) feet of patients in a health care setting; wear a respirator.
