Position Summary

The Department of Psychiatry seeks a Case Manager/System Navigator to work in/around WSESD to be responsible for developing and sustaining individualized treatment programs for at-risk adolescents in the Prevent 2 Protect Adolescent Targeted Violence Prevention Program. This is a community outreach endeavor aimed at providing intensive individualized supports to students, schools, and communities throughout the state.

The regional case manager will be responsible for planning, assigning, directing, reviewing, and evaluating resources, to develop and sustain individualized treatment programs for identified at-risk adolescents; coordinating community resources to provide for the optimal delivery of services for identified at-risk adolescents in the areas of education, mental health, juvenile justice, trauma, crisis response and, substance use in preparation for community integration, and living; coordinating with schools and educational entities to ensure progress toward educational achievement goals, graduation/certification/GED requirements and/or vocational skill acquisition; working in collaboration with regional mentors arranging services which meet the social, emotional, mental, and physical health needs and goals for identified at-risk adolescents; providing professional and technical assistance to Regional Mentors, addressing issues, resolving problems and making specific suggestions for action;  facilitating daily meetings with regional mentor to provide support and obtain progress reports for each case; providing daily reports to Program leadership on the at-risk adolescents, including, progress toward identifying/securing services for them and their families; assisting program leadership in preparing reports for program sponsors and other key stakeholders; and ensuring integrity and effectiveness of such services and compliance with federal and state rules and regulations.

This position is based in the West Shores Educational Service District (WSESD) working with children/adolescents in WSESD and the surrounding areas. The position requires daily travel throughout WSESD, occasional travel to East Lansing to meet with team members and daily/weekly virtual team meetings. The position is an off-date position funded through September 30, 2027, with possibility of extension based on the availability of grant funds.

Responsibilities of the position:

  • Coordinates community resources to provide for the optimal delivery of services for identified at-risk adolescents in the areas of education, mental health, juvenile justice, trauma, crisis response and intervention, substance use in preparation for community integration, and living
  • Coordinates with schools and educational entities to ensure progress toward educational achievement goals, graduation/certification/GED requirements and/or vocational skill acquisition
  • Works in collaboration with regional mentors arranging services which meet the social, emotional, mental, and physical health needs and goals for identified at-risk adolescents
  • Provides professional and technical assistance and training to Regional Mentors, addressing issues, resolving problems and making specific suggestions for action
  • Facilitates daily meetings with regional mentor to provide support and obtain progress reports for each case
  • Provides daily reports to Program leadership on the number of at-risk adolescents and progress toward identifying/securing services for them and their families
  • Assists program leadership in preparing reports for program sponsors and other key stakeholders

Unit Specific Education/Experience/Skills

Knowledge equivalent to that which normally would be acquired by completing one or two years of graduate study beyond a Bachelor's degree, such as a Masters in Social Work; one to three years of related and progressively more responsible or expansive work experience in counseling in a health/mental health environment; or an equivalent combination of education and experience; and State certification as a Social Worker. ACSW may be required in order to provide primary supervision of social work students.

Desired Qualifications

  • A license (LSW, MSW, LBSW, LMSW, LPC, LLC, LMHC, NCSP, EDS) to practice in the state of Michigan or licensure within 6 months of hire date
  • Demonstrated experience working in or with K-12 schools
  • Training in crisis intervention, risk assessment, threat assessment and/or working within a behavioral assessment team
  • Excellent organizational, interpersonal, coaching and communication skills to effectively work within a multi-disciplinary team setting
  • Demonstrated critical thinking and problem-solving skills
